Description
Healthy Families provides intensive home visits to support
new and expectant parents in the areas of child health and development,
positive parenting skills and a safe home environment. Families are
encouraged to identify and build on their existing skills and resources
and are assisted in establishing a network of community servies tailored
to their individual situation.
Participation
Healthy Families program participation is initiated on a referral-only
basis generally by a health, school or WIC professional. Home visits
are provided weekly during the first six to 12 months of the infant’s
life and then less often as necessary, up to the child’s fourth
birthday.
Eligibility
Healthy Families eligibility is based on the presence of risk factors
related to a child’s well-being in terms of a safe and nurturing
home environment and healthy physical and emotional development.
Eligible
parents must be first- or second-time parents of an expected child or
a child up to three months of age at the time of referral. Participation
is not based on income and is limited to residents of Carver or Dakota
counties.
Partnership
Health Families is a partnership among several agencies, organizations
and health care providers serving Carver or Dakota counties.
Cost
Healthy Families services are provided at no cost to eligible families.
